Unless specifically refused by you or your veterinarian, our pharmacy has the option to substitute the generic equivalent of prescribed drugs when available.

The Following is required information:
- Farm name, address, phone number
- Veterinary clinic name, address, phone, email, fax number
- Veterinarian name and license number
- Product name, strength, size and quantity
- Refills
- Dosing instructions
- Prescription expiration date
- Group of animals to be treated
- Appropriate Withholds
- Veterinarian signature and date
